Begin with your own details: your name, your email address and a password.
This first form creates your private account and nothing else. Your child’s details are entered only once you have confirmed your email address, signed in, and given your explicit consent for information about their health, development or disability to be used — all inside your own secure area.
There is nothing to pay at this stage. Payment of £75 is only requested after you have completed your questionnaire and are ready to submit it for review.
Your answers save as you go, so you can leave and return through your private account and complete the questionnaire in stages. It may help to have recent school reports, meeting notes, examples of work or assessment information nearby, but these are optional. Your own account of what you are noticing is the most important part. The questionnaire usually takes around 40 minutes in total.
